SCHOOL
DIRECTORY FORM
This form is required of all students. Information completed will be included
in an all-school directory organized by class. This directory will be
a great tool to help you stay connected with your child's classmates.
Completed forms must be received by Wednesday, October 24, 2007 for inclusion
in the 2007-2008 directory.

CLASSROOM
SUPPLY REQUEST FORM
To be used by teachers to request basic items like crayons, markers, glue
sticks, etc. to replenish their classroom supplies. Requests may also
be emailed to supplies@saratogapto.org.
WISH
LIST UPDATE FORM
Teachers may complete this form to create or update their wish lists.
Updates may also be emailed to wishlist@saratogapto.org.
FUNDING
REQUEST FORM
For teachers and staff to request funding for an academic school program,
activity or resource. See form for additional information and deadlines.
